Please follow the instructions below for collecting and shipping your child's biosample collections. If you have multiple children, you will receive a stool and saliva kit for each. Each kit has instructions about collecting the sample from your child. If you need help, have any questions, or would like to schedule a virtual appointment to walk through the sample collection, please contact us.
To prepare the return package that will be sent to the lab:
- Use toilet paper (not included) and alcohol wipe (included) to thoroughly clean the outside of each tube.
- Place each tube into the separate bio-specimen bag. DO NOT remove the absorbent material from the bio-specimen bag.
- Fold the brown cardboard box.
- Place the biospecimen bags with the tubes inside the cardboard box.
- Close the box.
- Insert the cardboard box into the FedEx ClinicalPak. Wrap the ClinicalPak around the box and seal tightly.
- Attach the return label to the outside of the ClinicalPak.
- Drop off the package at the nearest FedEx.
- This return label has been prepaid by the study team.

- Saliva (swab) sample collection kit
- White box with the stool collection kit, including a tube with a purple cap, a spatula, package of 2 toilet accessories
- 2 bio-specimen bags
- Small brown cardboard box
- FedEx ClinicalPak
- Prepaid return label
- Alcohol wipe

As a thank you for your participation, you will receive $75 for each sample from a study-eligible child. Once we have processed the samples, you will receive a confirmation (via text or email) and your payment will be issued. Please allow 2-3 weeks for processing.
